No State Payroll Totals

Problem

State totals not showing up on W-2 form printed from MacNail construction payroll software. Some items not showing up on payroll files when using 'update payroll files'.

Diagnosis

Did they first get payroll blank in in 1991 or earlier?
Early payroll blanks are lacking some defined names that tell it where certain tax totals are located.
This may conceivablely be a problem with some '92 blanks, but it's much less likely to be a problem with them.

Solution

Easiest solution is to send them a new 93 payroll blank and have them trash all the current payroll files and start over with the new blanks. In theory we should charge $20 for this but if you want to be nice and send it no charge, go ahead.
Can also define name on the payroll files and/or payroll blank. The total cell under the MA tax (or other state tax) column should be defined as state.total. Medical deductions are med.total, 'other' deductions are other.total, Disability is dbl.total, Local taxes are local.total

The new 3.4 'update payroll file' command, unlike previous versions, REQUIRES certain defined names so it can get the right values.
'93 payroll blanks DEFINITELY have all required defined names
'92 blanks SHOULD have all required defined names (unless we slipped)
'91 blanks PROBABLY will be missing some defined names
'90 blanks will definitely be missing all defined names and should NOT be used
